연구목적: 본 연구는 청소년 건강증진을 위한 보건의료서비스로 추진되고 있는 청소년 친화적 서비스(Youth-Friendly Service, YFS)를 이해하고 그 효과를 분석한다.
연구의 중요성: 기존 연구에서 부족했던 부분인 YFS에 대한 이해관계자들의 인식과 이용경험에 대해 연구하여 YFS에 대한 심층적 이해와 효과성을 높이기 위한 방안을 모색한다.
연구방법론: 연구 방법으로는 에티오피아 오로미아 지역에 속한 두 보건소를 방문하여 YFS 보건의료서비스 제공자, 또래 교육자, 센터 방문자, NGO 프로그램 담당관을 대상으로 포커스 그룹 인터뷰와 심층 인터뷰를 실시하였다. 세계 보건기구에서 제공한 청소년 친화적 서비스의 5대기본 원칙을 기준으로 변화 이론 접근법을 적용하여 서비스 효과를 분석하였다.
연구결과: 본 연구 결과 에티오피아의 YFS는 접근성, 수용성, 형평성, 적절성, 효과성 측면에서 전반적으로 긍정적인 평가를 받고 있었다. 다양한 프로그램과 서비스 제공을 통해 지역주민의 인식이 제고되었고 청소년들이 통합적인 의료 지원을 받는 것으로 나타났다. 그러나 서비스제공자의 과도한 업무, 보건소 인력의 높은 이직률, 또래 교육자의 비전문성, 지역을 고려한 예산 배분이 되지 않는 등이 YFS의 한계로 나타났다.
결론 및 시사점: YFS는 청소년들로 하여금 일반적인 보건의료서비스에 대한 이용 한계를 극복하고 적절한 서비스를 제공받을 수 있도록 지원하고 있는 것으로 나타났다. 효과적인 YFS의 확산을 위해 본 연구에서 밝혀낸 YFS에 대한 긍정적 요소들은 극대화하고 한계점들은 극복하여 청소년들이 보다 나은 성 건강 보건의료서비스를 받을 수 있도록 지원하는 것이 필요하다고 사료된다.

Purpose: Youth-Friendly Service (YFS) has been recognized as an important avenue for providing proper health care service and information to young people. This study aims to understand YFS and analyze the effectiveness of the service.
Originality: This study investigates the perception and experience of stakeholders about YFS programs, which have been understudied, and seeks to provide suggestions for YFS enhancement.
Methodology: A qualitative case study was conducted by interviewing health service providers, peer educators, YFS visitors, and NGO program officers at two YFS sites located in the Oromia region in Ethiopia. The data were analyzed with the YFS’ assessment tool, which was adapted from World Health Organization’s guidelines, while applying a theory of change approach.
Result: In the study, the selected YFSs at the two centers were positively assessed on the basis of five principles:
accessible, acceptable, equitable, appropriate, and effective. By conducting various activities and providing adolescent-friendly services, the awareness of YFS among community members has increased, and adolescents have begun receiving comprehensive health services, including sexual and reproductive health services. However, several limitations were also found, such as the health service providers being overworked because of a lack of personnel, a high turnover of staff at the health center, limited knowledge of peer educators, or a lack of consideration of regional differences such as population or program size when allocating cost.
Conclusion and Implication: To enhance adolescents’ sexual and reproductive health, it is necessary to maximize the positive elements of YFS that were presented in this study and find a way to overcome the limitations through the collaboration of various groups including adolescents, health service providers, community members, NGOs, and government officials.
