Solar lentigines and ephelides removal in Asians using a combination of a novel 670 nm picosecond and 1,064 nm laser

Background: Picosecond (PS) lasers have evolved over the years, and several new wavelengths have been adopted for more efficient and safe treatment of various skin conditions. We specifically studied the 670 nm wavelength for its high melanin-hemoglobin ratio, effectiveness, and safety in laser-unfriendly Asian skin. Objective: This study evaluated the effectiveness, safety, and patient satisfaction following treatment of solar lentigines and ephelides with a combination of a novel 670 nm PS and 1,064 nm wavelength laser in a series of Asian patients. Methods: This retrospective study evaluated 30 patients who underwent treatment for solar lentigines and ephelides. A PS laser of wavelength 670 nm and 660 PS pulse width was adopted at a spot size of 2.0–3.5 mm, fluence of 0.7–0.8 J/cm2, and repetition of 2.0–3.3 Hz. Treatment intervals ranged from 3–5 weeks for a total of 3–6 sessions. Skin pigmentations in the patients were documented with a high-resolution camera and the results were assessed by physicians. In addition, patient satisfaction was evaluated. Results: Thirty patients (27 females, 3 males), aged 21–66 years, with Fitzpatrick skin type III or IV were treated and evaluated. It was observed that 100% of the patients achieved at least good results (>50% improvement), 80% achieved good results (>75% improvement), and 40% achieved clinical clearance (>90% improvement). The average downtime with mild redness was 2–3 days and transient darkening of pigmentation (crème brulee reaction) was observed for 5–7 days. Higher-contrast lesions demonstrated better responses. The post-inflammatory hyper- and hypopigmentation rates were 3.3% and 6.67%, respectively. Conclusion: PS laser is effective for the treatment of solar lentigines and ephelides. The 670 nm wavelength is beneficial in improving the efficacy and safety of the PS laser treatment, which is especially important in Asian skin that is prone to laser complications.

Background: The emphasis on non-face-to-face or untact operation services is on the rise due to the ongoing widespread of COVID-19 epidemics, which have caused severe damage worldwide. Untact technology has been applied not only to everyday life but also to the beauty art industry that relies on customer service with contact. Objective: A safe non-permanent tattoo ink is necessary to overcome the stigma of permanent tattoos and tattoo removal procedures. The purpose of this study is to efficiently deliver non-permanent tattoo ink into the skin through sophisticated untact printing techniques and safe silver compound ink, further minimizing the side effects due to unsanitary conditions. Methods: Silver-gelatin compound ink serves as an excellent alternative for conventional permanent tattoos with additives. Pistontype extruder (PTE) system and internet of things (IoT) integrated precision-controlled non-permanent and untact system using silver nanoparticle ink. Results: Complex and sophisticated designs were three-dimensional (3D) printed using a non-permanent tattoo ink containing an optimum concentration of Ag+ under 5 N compression force through a 100 μm radius nozzle and diffused up to 200 μm into the stratum corneum through skin contact. Intradermal diffusion simulation and disappearance of the ink within two weeks of the human skin replacement cycle were successfully demonstrated. Conclusion: The integration of IoT and 3D printers has enabled a hygienic untact tattoo printing technique that has been verified through repeated testing. This study could arouse positive possibilities and interests in the rapidly changing beauty art fields. It provides a new tattoo methodology and further research ideas for the 3D printing applications in tattoo production.

A medical perspective of the practice of semi-permanent makeup in South Korea

Background: Despite existing problems with infection control and healthcare regulations associated with semi-permanent makeup, its practice by non-medical personnel at non-healthcare institutions continues to pose fatal risks to the safety of the community. Objective: The aim of this study is to seek for improvement measures on the Korean legal system regarding the practice of semipermanent makeup by investigating current conditions of the procedure and materials used, reviewing past clinical reports of complications associated with tattooing, and assessing domestic and foreign laws and regulations of semi-permanent makeup. Methods: Both domestic and foreign studies and publications on the regulatory system with regards to semi-permanent makeup, in addition to clinical cases of complications associated with semi-permanent makeup, were reviewed. Results: Tattoo procedures in South Korea and Japan are strictly restricted to medical personnel, while other countries also regulate the practice of tattoo with a license-based system rather than a qualification-based system such that non-medical personnel can perform under medical supervision. The duration and placement of pigments in the dermis of the skin and its possible complications associated with the procedure have been confirmed; short-term side effects are bleeding, infection, allergic reaction and Koebner phenomenon, and long-term side effects are foreign body reaction, granuloma. Local infections caused from tattoos include warts, herpes simplex; systemic infections include hepatitis B and C, HIV; toxic immune reactions include erythema multiforme, vasculitis, granuloma. To address these issues, examination of patient history for potential indications, established protocol for medical waste management, and stringent regulations for hygiene product management are necessary. Conclusion: Measures such as the establishment of a license-issuance system that is inclusive of education and training on the proper practice of semi-permanent makeup and stricter management of tattoo-related materials and devices should be enforced in order to ensure infection control and the safety of the public health.

The effect of makeup behavior and makeup perception on makeup usage level before and during the COVID-19 pandemic

Background: This study investigated the impact of perceptions on makeup behavior and levels of makeup use before and during the COVID-19 pandemic. Objective: It is presumed that during the COVID-19 outbreak, due to the restrictions on meeting people, mandatory wearing of facial masks, and the spread of contactless culture, there may have occurred a change in the makeup behavior, makeup perception, and level of makeup use. However, no empirical study has been conducted on this topic. Hence, this study aimed to identify the effect of the COVID-19 pandemic on makeup use in female adults, using an empirical analysis method. Methods: A survey was conducted on 245 female adults aged 18 years or older, residing in the Gwangju Metropolitan City. Empirical analysis was performed at a significance level of p<0.05, using the SPSS WIN 21.0 program. To identify general characteristics of survey respondents, a chi-squared test was performed, and to verify the feasibility of estimation tools, a factor analysis was conducted. To measure the level of reliability, Cronbach’s α coefficient was calculated. A paired samples t-test was used to determine the level of makeup use before and during the COVID-19 pandemic. Lastly, a multiple linear regression analysis identified the impact of makeup behavior and makeup perception on the makeup usage level before and during COVID-19. Results: The analysis of difference in makeup usage before and after the pandemic began showed that makeup usage during the pandemic (mean=2.84) decreased from the pre-pandemic level (mean=3.59). It was also found that higher aesthetics, synchronism, and courtesy as subfactors of makeup behavior led to an increase in makeup usage during the pandemic. In addition, this study also confirmed that respondents with higher social achievement as a subfactor of makeup perception increased their makeup usage during the pandemic. Conclusion: Overall, makeup usage during COVID-19 decreased compared to the pre-pandemic level, but respondents with higher aesthetics, synchronism, and courtesy, which are subfactors of makeup behavior, and those with higher social achievement, a subfactor of makeup perception, tended to wear more makeup after the pandemic began. These findings may be used as basic data for cosmetic marketing.

Skin tightening by an insulated microneedle radiofrequency device combined with lower blepharoplasty: a randomized fellow eye comparison study

Background: Aging in the lower eyelids creates bags under the eyes from the protruding orbital fat, and periorbital wrinkles. Therefore, lower blepharoplasty should be combined with a skin tightening procedure to enhance the results. Objective: To compare the effects between lower blepharoplasty combined with an insulated microneedle radiofrequency (RF) device and lower blepharoplasty alone. Methods: Twelve Asian (24 eyes) patients with baggy eyelids were included. One eye per patient was randomized for lower blepharoplasty combined with insulated microneedle RF devices (RF group), and the other eye for lower blepharoplasty alone (LB group). All patients underwent transconjunctival lower blepharoplasty with fat transposition and orbicularis oculi suspension. The primary outcome was the difference between both groups on the five-point Wrinkle Assessment Scale (WAS), assessed at baseline and at 2 months post-surgery. The secondary outcome was patient satisfaction using the Global Aesthetic Improvement Scale (GAIS). Results: The WAS improvements were 75.0% (9/12 eyes) and 58.3% (7/12 eyes) in the RF and LB groups, respectively. The GAIS showed high patient satisfaction in both groups, with an average of 4.12 (3-5) in the RF group and 4.0 (2-5) in the LB group (p=0.157). There was no difference in the postoperative inflammation between both eyes at 1 week; and no decrease in the WAS score at 2 months or any adverse effects at 6 months were observed. Conclusion: Wrinkle improvement was observed post-lower blepharoplasty with or without microneedle RF treatment. The insulated microneedle RF device proved safe when performed concurrently with lower blepharoplasty.

Outcome of mid- and lower face lifting using bidirectional cone sutures at 6 months and 1 year

Soft tissue ptosis occurs with age, leading to loss of facial volume. The facial retaining ligamentous system is an intricate network of fibrous tissues connecting the whole face, restraining soft tissues from gravitational forces, and opposing facial movements. With proper positioning and anchorage within the ligamentous grid and superficial fat repositioning, lifting the mid-face and adjacent areas (e.g., jowl) is likely to occur. We aimed to determine whether inserting 3 pairs of bidirectional cone sutures can effectively improve the nasolabial fold (mid-face) and jowl (lower face) in Asian patients with mild to moderate mid-face laxity at 6 months and 12 months. Ten healthy volunteers with mild to moderate facial laxity received 3 pairs of bidirectional cone threads, inserted in a straight parallel technique. Based on validated evaluation parameters: facial laxity rating scale (FLRS) and the wrinkle severity rating scale, a total of 7 independent reviewers, among which 3 assessed the change in mid-face laxity (nasolabial fold), and the other 4 reviewers evaluated the change in jowl or lower face contour based on FLRS at 6 months and 12 months. All reviewers were experienced esthetic doctors. According to the reviewers’ evaluation based on the validated parameters, there was a linear improvement in the mid-face laxity/nasolabial fold and lower face laxity/jawline contour self-reporting patients’ satisfaction at 6 months and 12 months compared to baseline in all 10 patients. There was continuous improvement in mid-face laxity and lower face laxity from baseline, 6 months, and 12 months after treatment. The evaluation results among the reviewers were similar, apart from reviewer 7. All patients gave their best rating at 12 months, and their satisfaction almost doubled compared to that before treatment. Level of Evidence: Level IV, brief communication

Arteriovenous malformation (AVM) is a vascular malformation with an abnormal capillary shunt between the arterial and venous systems. AVMs can arise anywhere on the body, and are occasionally located in the head and neck. However, AVMs of the nose are considerably rare. Most cases of AVMs of the nose have no symptoms, but some cases show epistaxis and nasal obstruction. Furthermore, a differential diagnosis between hemangioma, lymphangioma, and AVM is important. In cases of AVMs, especially on the face, surgeons should consider cosmetic factors and choose an appropriate plan of treatment. We report the case of a 22-yearold male who presented with an asymptomatic, palpable mass on the nasal dorsum and was successfully treated with transcatheter arterial embolization and sclerotherapy.

Functional and esthetic reconstruction of microtia and congenital aural atresia: a case report

Reconstruction of microtia and congenital aural atresia is a challenging surgery that requires experience and thorough knowledge. The goals of auricular reconstruction and atresiaplasty are to improve the cosmetic appearance and rehabilitate the acoustic function. There are several options for reconstruction of the external ear. Here, we present a case of microtia and congenital aural atresia in an 18-year-old woman. The two-stage auricular reconstruction using autologous costal cartilage and atresiaplasty were performed. The first stage of auricular reconstruction included fabrication and placement of the costal cartilage framework in the subcutaneous pocket. The second stage of auricular reconstruction involved elevation of the cartilage framework and a split-thickness skin graft. In the third stage, atresiaplasty involved reconstructions of the external auditory canal and tympanic membrane. Autologous microtia reconstruction with atresiaplasty is a good option to achieve improving the cosmetic appearance and acoustic function in patients with microtia and congenital aural atresia.

A majority of alar rim deformity cases arises from the procedural complications of rhinoplasty. However, an increasing interest in the aesthetic aspects of alar rim deformity has led patients with congenitally retracted or hanging alae to seek revision surgeries. Alar retraction is predominantly caused by an excessive cephalic orientation or overmedialization of the lower lateral cartilage and a lack of soft tissue around the alar rim. The aim of this article is to present the case of a 59-year-old female patient with retracted alae from cephalically malpositioned lower lateral cartilages. We addressed these by using an external rhinoplasty approach to separate lower lateral cartilage from the surrounding tissues and apply an alar spreader graft and intergrated lateral crural strut graft to effectively correct the nostril show without any significant adverse effects.

Deviated nose correction, a challenging procedure, requires a strategic approach to achieve better results. The Plan-Do-Check-Act (PDCA) cycle can be adapted as a strategy. Three-dimensional (3D) scans and computer simulations enhance planning. Anterior septal reconstruction and support with titanium miniplates are helpful in correcting nasal and septal deviation correction. During surgery, checking with a customized 3D printing rhinoplasty guide is essential for realizing the simulated nose shape. A detailed assessment of nose shape and subjective evaluation of patient satisfaction are necessary during the follow-up period for continuous improvement.
